Responsibilities

The Proxy Care Nurse is responsible for the overall implementation, maintenance, delivery and coordination of a proxy care program for assigned communities. This individual also assists in monitoring the medication management program.

 

  • Oversee and manage all aspects of the proxy care giver program as per Georgia regulations. This includes new hire and ongoing proxy care training.
  • Along with the Resident Services Director (RSD) and the Life Guidance Director, responsible for obtaining required documentation for all residents requiring proxy care services.
  • Maintain in an organized fashion the appropriate documentation for residents’ proxy care services and proxy care staff training as required by the state.
  • Provide ongoing support and encouragement to proxy care staff.
  • Maintain positive relationships with residents, families, physicians, and staff in assigned communities.
  • Demonstrate flexibility by being open to change and responding to change with a positive attitude.
  • Prioritize tasks to assure that the customers’ and the communities’ changing needs are met in a timely manner.
  • Identify opportunities to improve and promote quality.
  • Monitor the medication program in assigned communities for any unsafe practices and report issues to the RSD or Executive Director in the community.
  • Able to present proxy care program in a positive manner to ensure compliance with staff, residents, physicians and families.
  • May perform other duties as needed and/or assigned.
Qualifications
  • Registered Nurse with a current Georgia RN license in good standing.
  • Must successfully complete Atrias Medication Management Training.
  • Must successfully complete all Atria specified training programs.
  • Able to read and comprehend complex instructions, correspondence, and memos. Able to write correspondence. Able to effectively present information in oneonone and small group situations to customers, clients, and other employees of the organization.
  • Able to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Able to apply common sense understanding to carry out detailed written or oral instructions. Able to deal with problems involving a few concrete variables in standardized situations.
  • Able to work various schedules and shifts as needed.
  • Proficient in interpreting state regulations.
